Important Information
- Amendments: all revisions, additions or deletions to approved studies
- All amendments must be submitted via CAPCR for review to ensure that the research remains scientifically and ethically sound.
- If your submission requires a Full Board review, please consult the submission deadline dates found here to determine the appropriate date.
- The Principal Investigator (PI) is responsible for ensuring that amendments are submitted to the REB for review.
- REB approval must be received prior to implementation.
Types of Amendments

Making a Change
- Complete the amendment via CAPCR
- Include all documents affected by the amendment
- Revised Protocol (tracked and clean copies), Informed Consent Form(s), IB, etc.
- Supporting documents (i.e. new information supporting amendment)
- Ensure version dates are updated for documents that are updated
- Retain a copy for your records.
